A provider-patient relationship may only be established after completion of the required intake process, consent forms, identity and location verification, and consultation with an appropriately licensed healthcare provider.

a. Booking

Submitting a booking request does not guarantee that an appointment or service will be provided. We reserve the right to decline, cancel, or reschedule appointments based on provider availability, eligibility, safety, or clinical considerations.

b. Cancellations and No-Shows

Cancellation, rescheduling, deposit, and no-show requirements will be disclosed during booking or in your appointment confirmation. Appointments cancelled without sufficient notice or missed without notification may result in a cancellation or no-show fee.

c. Appointment Preparation

You may be required to complete intake forms, provide identification, disclose your current location, submit medical records, or complete laboratory testing before your consultation. Late arrivals may result in a shortened appointment, cancellation, or rescheduling.

Some Ocean Wellness services are provided through video calls, telephone calls, electronic forms, secure messaging, or other telehealth technologies. By participating in telehealth services, you understand that:

• Telehealth may have limitations compared with an in-person examination.
• Technical issues may interrupt or delay a consultation.
• A provider may require additional testing or an in-person evaluation.
• You are responsible for using a private location and secure internet connection.
• You must not record consultations without prior permission.

A separate telehealth consent form may be required before services are provided.

Payment may be required before a consultation, laboratory service, assessment, membership, product, or other service is provided. By providing payment information, you authorize Ocean Wellness and its payment processors to charge the applicable fees.

Prices may vary depending on your selected program, provider recommendations, laboratory requirements, medications, supplements, and other services. Prices may change for future services without prior notice.

You are responsible for any fees charged by your bank, credit card provider, financing provider, or payment processor. Refunds, where available, will be governed by the refund or cancellation policy disclosed at the time of purchase.

Unless expressly stated otherwise, Ocean Wellness services may not be covered by health insurance, Medicare, or Medicaid. You are responsible for confirming whether your insurance provider offers reimbursement for consultations, laboratory testing, prescriptions, or other services.

Ocean Wellness does not guarantee insurance coverage or reimbursement. You remain responsible for all applicable fees regardless of whether your insurance provider reimburses you.

Before receiving services, you may be required to:

• Complete a health and medical-history questionnaire.
• Disclose current medications, supplements, allergies, and health conditions.
• Provide laboratory results or other relevant records.
• Review and sign informed-consent documents.
• Confirm your identity and physical location.

You represent that all information you provide is truthful, current, and complete. Incomplete or inaccurate information may affect the safety and effectiveness of your care. Ocean Wellness is not responsible for outcomes caused by information you intentionally fail to disclose or provide inaccurately.

Ocean Wellness may coordinate laboratory testing, specimen collection, or related services through independent laboratories and licensed service providers. You are responsible for following all preparation, collection, storage, and shipping instructions.

Testing or processing may be delayed due to insufficient samples, collection errors, shipping delays, laboratory issues, weather, or circumstances outside our control. Laboratory results should not be used for self-diagnosis or independent changes to your medications or treatment plan. Results should be reviewed with a qualified healthcare provider.

All clinical decisions are made by licensed healthcare providers using their independent professional judgment. Completing an assessment or consultation does not guarantee that you will receive a prescription, medication, supplement, laboratory order, or particular treatment recommendation.

A provider may determine that:

• A requested service is not clinically appropriate.
• Additional testing or records are required.
• An in-person evaluation is necessary.
• You should seek care from another healthcare provider or specialist.

Any prescription medication must be used only as directed by the prescribing provider and dispensing pharmacy.

Certain medications recommended through Ocean Wellness may be compounded by licensed pharmacies or prescribed for an off-label use when a licensed provider determines that doing so is clinically appropriate. Compounded medications are not reviewed or approved by the FDA in the same manner as commercially manufactured, FDA-approved medications.

The prescribing provider or pharmacy will provide applicable information regarding potential benefits, risks, alternatives, dosage, and proper use.

Individual results vary based on many factors, including age, medical history, genetics, lifestyle, nutrition, physical activity, sleep, health conditions, medication use, and adherence to provider recommendations.
